This area of the web site gives you the opportunity to print off standard forms to fill out and bring with you to the office. This allows our staff to serve you better and make your visit time more efficient. By clicking on the New Client or New Patient Form you can fill out a form that often takes 10-15 minutes to fill out here. This information must be added to the computer before Dr. Butler can see your pet. The Dermatology Form allows you to provide historical and treatment information on your pet's skin problem. This form may be required by Dr. Butler especially in cases of chronic or long term problems. The Pet Drop Off Form gives us necessary information about a current problem in your pet that you are going to drop off on your way to work or other appointment.
